Intake Forms let you collect additional information from customers during the booking process.
Appointy already collects standard customer details such as:
- First name
- Last name
- Phone number
- Email address
Use an Intake Form when you need to collect extra information related to the appointment, such as preferences, medical details, instructions, or other service-specific information.

What happens after you enable Intake Forms?
Enabling Intake Forms makes it possible to collect additional customer information during booking.
You can then:
- Create intake form fields
- Choose the type of response required
- Mark fields as mandatory
- Assign fields to specific services
- Reorder intake form fields
- Automatically fill previously saved customer information for returning customers
Example
Suppose you run a consultation-based business.
Appointy already collects the customer's basic contact details, but you may also want to ask:
- Reason for Visit
- Preferred Contact Method
- Medical History
- Allergies
- Additional Notes
These questions can be added using Intake Form fields.
Intake Forms can be service-specific
You do not need to show the same intake questions for every service.
Each intake form field can be assigned to the service or services where it is relevant.
For example:
Initial Consultation
- Medical History
- Allergies
- Reason for Visit
Follow-up Consultation
- Current Symptoms
- Additional Notes
This helps keep the booking form relevant and avoids asking unnecessary questions.
