In Appointy, you can create service-specific intake forms by assigning individual intake form fields to the services where they should appear.
This lets you collect different information for different services instead of showing the same questions for every booking.
For example, a consultation may require medical-history questions, while a regular follow-up appointment may only need a short notes field.

Add multiple fields to build a service-specific intake form
An intake form for a service is created by assigning the required fields to that service.
For example, an Initial Consultation may include:
- Reason for Visit
- Medical History
- Allergies
- Current Medication
- Additional Notes
You can assign each of these fields to the same service to build the complete intake form.
Assign the same field to multiple services
You do not need to create the same field separately for every service.
One field can be assigned to multiple services.
For example:
Emergency Contact
can be assigned to:
- Initial Consultation
- Therapy Session
- Health Assessment
This keeps your intake form setup easier to manage.
Create a new field if required
If the question you need does not already exist, create a new intake form field first and then assign it to the relevant service or services.
Service-specific intake forms
Different services can have different combinations of intake form fields.
For example:
Initial Consultation
- Medical History
- Allergies
- Reason for Visit
Follow-up Consultation
- Current Symptoms
- Additional Notes
This lets you collect only the information relevant to each booking type.
