Either of the two routes given below can be followed to remove a field from the intake form of a service.
Option 1
Quick Nav — (Settings → Intake Form → Mouseover on field that you want to remove from the intake form of a service → Click on the pencil-like edit icon that appears → In the ‘Update field’ panel, deselect the service to remove this field → Click on ‘Update’ → Click on ‘Save Changes’)
- Log into the Appointy admin panel.
. - Click on the ‘Settings’ icon in the toolbar appearing on the left.
. - Go to the ‘Intake Form’ tab under the ‘Customization’ section.
. - In the ‘Intake Form’ window, mouseover on the field that needs to be removed from the intake form of a service and click on the pencil-like edit icon that appears.

. - In the ‘Update field’ side panel, deselect the service(s) from which the field needs to be removed.

. - Click on the ‘Update’ button in the ‘Update field’ side panel and then on ‘Save Changes’.

Quick Nav — (Settings → Service → Click on the service name for which an intake form field needs to be removed → Go to the ‘Intake Form’ tab → Mouseover on the field that needs to be removed → Deselect the service in the ‘Update Field’ side panel → Click on ‘Update’ → Click on ‘Save Changes’)
- Log into the Appointy admin panel.
. - Click on the ‘Settings’ icon in the toolbar appearing on the left.
. - Go to the ‘Service’ tab under the ‘Resource Management’ section.
. - Select the service name for which an intake form field needs to be removed and go to the ‘Intake Form’ tab.
. - Mouseover on the field that needs to be removed and click on the pencil-like edit icon that appears.

. - In the ‘Update field’ side panel, deselect the service(s) from which the field needs to be removed.

. - Click on the ‘Update’ button in the ‘Update field’ side panel and then on ‘Save Changes’..
Note: If you delete the field, it will be deleted for all the services to which the field has been assigned in the intake form.
