Here’s how you can disable an intake form for a service:
Quick Nav — (Settings → Service → Select the service for which you want to disable the intake form → Click on ‘Intake form’ tab → Mouseover on the intake form field and click on the pencil-like edit icon → In the ‘Update Field’ side panel, deselect the service, click on ‘Done’ and then on ‘Update’ → Click on ‘Save Changes’)
- Log into the Appointy admin panel.
. - Click on the ‘Settings’ icon in the toolbar appearing on the left.
. - Go to the ‘Service’ tab under the ‘Resource Management’ section.
. - Click on the service name for which you want to disable the intake form.
. - Mouseover on the intake form field and click on the pencil-like edit icon.

. - In the ‘Update Field’ side panel, click on the service name appearing below and deselect the service from the ‘Select Service’ side panel and click on ‘Done’.

. - Click on ‘Update’ in the ‘Update Field’ side panel and then click on ‘Save Changes’.
Note: If you delete the field, it will be deleted for all the services to which the field has been assigned in the intake form. Therefore, we advise you to deselect all the fields one by one from the ‘Intake form’ tab of the service.
